🔒 Health Program Consent

Required by HIPAA — 45 CFR §164.508 — before collecting any health information

What you are consenting to

The JJ Center offers an optional health education program. If you choose to participate, you may share health topics of interest with our licensed health professionals to receive personalized video recommendations and content.

Your HIPAA rights

  • Participation is entirely voluntary — declining has no effect on food pantry access or any other service
  • You may withdraw consent and delete your health data at any time from your dashboard
  • Your health information will never be shared without your separate written consent
  • Only authorized JJ Center health professionals may access your health profile
  • You may file a complaint with HHS OCR at hhs.gov/ocr if you believe your rights were violated

How your health data is protected

  • Stored in a separate AES-256 encrypted database with restricted access
  • Covered by a HIPAA Business Associate Agreement with our technology providers
  • Retained 6 years per HIPAA requirement, then permanently deleted
  • Subject to annual compliance review each June by Dr. Roach and Admin
